Withdrawal Form

If you would like to withdraw from classes or company, please fill this form out at least ten days prior to the first of the month. Verbal notice or informal communication does not complete withdrawal. Forms received without the required notice are not eligible for refunds for tuition already billed within the current cycle.

*Withdrawal means ending your season contract prior to the end of May.
